Can you use tranexamic acid with hydroquinone?

Yes, with care

Yes, you can use tranexamic acid with hydroquinone, with care: clinics use them together for melasma, so the practical plan is tranexamic acid in the morning and hydroquinone at night, which needs no waiting time between them.1234

The chemistry

They reduce pigment by different routes. Hydroquinone blocks the enzyme that makes melanin, while tranexamic acid is thought to calm the signals that switch pigment cells on, and randomized trials show it lowers melasma severity (Calacattawi 2024). Head to head, a split-face trial in skin of color found 3% tranexamic acid cream and 4% hydroquinone cream performed similarly for mixed melasma (Yasnova 2024). Together, the evidence is thinner. In a retrospective study, 108 patients with complex facial pigmentation all received tranexamic acid and hydroquinone cream, and half also had light-based photorejuvenation. The light group responded better, 98% against 83% (Lin 2023). That shows the two drugs were used side by side, but it was not randomized and did not test the pair against either drug alone.

Chemistry

No known conflict between them.

Irritation

Hydroquinone is the likelier irritant; topical tranexamic acid is usually well tolerated.

Order and wait

Tranexamic acid in the morning, hydroquinone at night.

Common mistakes

People sometimes stop hydroquinone abruptly and switch to tranexamic acid alone, then see pigment return. Talk to your prescriber about a plan for tapering and maintenance. Another mistake is adding microneedling or peels at home to push tranexamic acid deeper while also using hydroquinone, which raises the risk of irritation. Treat those procedures as clinic decisions.

Your routine

Morning

  1. Gentle cleanser
  2. Tranexamic acid serum
  3. Moisturizer
  4. Broad-spectrum SPF 30+

Evening

  1. Gentle cleanser
  2. Hydroquinone as prescribed, on dark patches
  3. Moisturizer

Go slower if

Hydroquinone is best used under a doctor's supervision. Ask before using either in pregnancy (Bozzo 2011). This page covers topical tranexamic acid only; tranexamic acid tablets are a separate prescription decision.

Evidence strength

Moderate. Each has randomized-trial evidence for melasma (Calacattawi 2024, Yasnova 2024). The only study we found using both is retrospective and mixed in a light treatment (Lin 2023).

FAQ

Does tranexamic acid go before or after hydroquinone?

We suggest using them at different times: tranexamic acid in the morning, hydroquinone at night. If your doctor wants both at night, tranexamic acid serum first, then hydroquinone once it dries. No study has tested an order.

How long should I wait between tranexamic acid and hydroquinone?

Using them morning and night spaces them about twelve hours apart. No shorter interval has been studied; if you layer them, let the first dry fully.

Which works better on its own?

In a split-face trial in skin of color, 3% tranexamic acid cream and 4% hydroquinone performed similarly (Yasnova 2024).

Can tranexamic acid keep results after a hydroquinone course?

It is a common clinic plan, but it hasn't been tested as a maintenance step for this pair, so agree it with your prescriber.
