Can you use tretinoin with tranexamic acid?
The chemistry
Tretinoin speeds up the turnover of pigmented cells and has been used for melasma for decades; it is also a component of triple-combination creams, which have strong melasma evidence (Taylor 2003). Tranexamic acid reduces pigment signalling and has meta-analysis support for melasma (Calacattawi 2024). Tranexamic acid is non-acidic despite its name, so it layers easily. Triple-combination cream pairs tretinoin with hydroquinone and a mild steroid, and it remains a benchmark for moderate to severe melasma, but it is used for limited periods because of steroid side effects and hydroquinone concerns. Tranexamic acid is increasingly used as a maintenance option or as an alternative for people who cannot use hydroquinone (Yasnova 2024). Combining tretinoin with tranexamic acid therefore borrows the turnover effect of the triple cream and swaps in a gentler pigment agent. Because tranexamic acid works on a different pathway, it can add to tretinoin's effect rather than duplicate it.
Target
Melasma, dark marks.
Irritation
Mostly from tretinoin.
Order
Tranexamic acid first, then tretinoin, or morning and night.
Common mistakes
Many people use tranexamic acid in the morning under sunscreen and tretinoin at night. Irritation from tretinoin can worsen melasma, so start slowly. Melasma is easily worsened by irritation. If tretinoin makes your cheeks red and peeling, that inflammation can leave more pigment, not less. Start tretinoin at a low strength, such as 0.025 percent, two or three nights a week, and use a moisturizer. Tranexamic acid in the morning is usually comfortable even on sensitised skin. Sunscreen is non-negotiable: tinted products containing iron oxides block visible light, which matters for melasma. Results take three months or more. If you are considering oral tranexamic acid, which has stronger evidence, it needs a medical assessment because it affects blood clotting and is unsuitable for people with clotting risk.

- Tranexamic acid serum
- Sunscreen SPF 30+
Evening
- Cleanser
- Tretinoin
- Moisturizer
Go slower if
Avoid tretinoin in pregnancy.
Evidence strength
Strong for tretinoin in combination creams; moderate for topical tranexamic acid.
FAQ
Can I use tranexamic acid with microneedling?
Studies exist (Feng 2024), but this should be done by a professional.
Will tretinoin alone fade melasma?
It can help over many months, but alone it is usually slower and weaker than combinations.
Is topical tranexamic acid safe for people who cannot take the oral form?
Topical use leads to very little absorption into the bloodstream, so it is generally considered low risk, but tell your doctor about any clotting history.
