Can you use tretinoin with kojic acid?
The chemistry
Tretinoin is the best-studied topical retinoid for photoaging and is used in melasma treatment, partly by speeding up the shedding of pigmented cells (Mukherjee 2006). Kojic acid, derived from fungi, inhibits tyrosinase and reduces melanin production (Saeedi 2019). In theory, combining them addresses pigment from both ends. In practice, tretinoin's irritation is common in the first months, and kojic acid is a recognised contact sensitiser, with allergic dermatitis reported from cosmetics (Nakagawa 1995). Irritation and allergy can look similar, and if both start together you cannot tell which product is responsible. Inflammation can also darken pigment in melasma-prone skin, undoing the purpose of the routine.
Risk
Irritation and allergy.
Approach
One at a time.
Better-studied options
Azelaic or tranexamic acid have stronger pigment evidence.
Common mistakes
Start tretinoin first, two or three nights a week, building up over a month or two. Once your skin is comfortable, patch test a kojic acid product on a small area for a week, then use it in the morning or on alternate nights. Kojic acid oxidises in light and air, turning brown and losing effect, and derivatives like kojic dipalmitate are more stable but less studied (Zilles 2022). Be cautious with imported whitening creams that claim fast results, as some have contained undeclared steroids or mercury. If pigment is the goal, discuss azelaic acid or tranexamic acid, which have stronger evidence and fewer allergy concerns. Sunscreen use every day is essential, because tretinoin makes skin more sun-sensitive and UV fuels pigment. If you have melasma specifically, a dermatologist may prescribe a triple-combination cream containing tretinoin, hydroquinone and a steroid for a short course, which has much stronger evidence than kojic acid (Taylor 2003).
Your routine
Morning
- Cleanser
- Kojic acid serum
- Sunscreen
Evening
- Cleanser
- Tretinoin
- Moisturizer
Go slower if
Avoid tretinoin in pregnancy (Bozzo 2011). Patch test kojic acid and stop at the first sign of itchy rash.
Evidence strength
Strong for tretinoin; limited for kojic.
FAQ
Why is this pairing marked 'care' rather than 'yes'?
Because tretinoin's irritation and kojic acid's allergy risk can overlap, so introducing them one at a time is important.
What should I do if my face gets itchy and red?
Stop kojic acid first, keep tretinoin paused for a few days, and see a doctor if it spreads or swells.
Can I use kojic soap with tretinoin?
Soaps are drying; with tretinoin they often cause irritation. Use them sparingly.
