Can you use tretinoin with urea?
The chemistry
Tretinoin is prescription retinoic acid. Its prescribing information recommends a non-comedogenic moisturizer, or a moisturizer with sunscreen, every morning after washing. The same label warns that other topical medications, drying cosmetics and astringents should be used with caution because of possible interaction, and singles out sulfur, resorcinol and salicylic acid (tretinoin label). Urea is not named, so the label neither clears nor bans it. A clinical review explains why strength matters: urea at 10% or lower moisturizes, above 10% it becomes keratolytic, and it acts as a penetration enhancer in medicated creams (Celleno 2018). A low-strength urea cream therefore fits the morning moisturizer role the label describes, while a high-strength one behaves more like an extra exfoliant on skin tretinoin is already thinning. In a randomized trial on neck pigmentation, 10% urea cream and 0.025% tretinoin cream were tested against each other, and tretinoin worked better (Treesirichod 2021). That comparison tested each one alone, not the two combined.
Chemistry
No known reaction. The label does not list urea among products to avoid.
Irritation
Low at 10% or less; strong urea creams add exfoliation.
Order and wait
Tretinoin at night, urea moisturizer in the morning, so no timed gap.
Common mistakes
People often start tretinoin, peel, and reach for the strongest cream in the cabinet, which is frequently a 20% or 40% urea product made for heels. That adds a second exfoliant and can worsen the rawness. A second mistake is applying extra tretinoin to speed things up: the label says more than one dose a day brings no faster results and more irritation (tretinoin label). Finally, the label advises keeping tretinoin away from the eyes, mouth and corners of the nose, which are also the spots urea stings most. Keep strengths honest: face-friendly urea is usually in the lower single digits to low teens; body-strength 20% plus formulas are for thick, dry patches and sting more next to tretinoin (Celleno 2018). A common calm plan is tretinoin at night on dry skin, plain moisturizer, and urea on alternate nights or on body areas away from the tretinoin field. Renova-style waits still apply to your tretinoin tube; urea does not cancel them. Wait page: /how-long-to-wait-between-tretinoin-and-moisturizer/. Wider night order: /layering-order/.
Your routine
Morning
- Gentle cleanser
- Urea moisturizer, 10% or less
- Broad-spectrum SPF 30+
Evening
- Gentle cleanser, pat dry
- Tretinoin as prescribed
- Plain moisturizer if your prescriber agrees
Go slower if
Tretinoin is prescription-only and should be avoided in pregnancy unless your doctor approves. The label calls for utmost caution on eczematous skin. Stop and call your prescriber if skin blisters, crusts or swells.
Evidence strength
Limited. The routine rests on the tretinoin label's moisturizer advice and a clinical review of urea (Celleno 2018). The only trial with both ingredients compared them separately (Treesirichod 2021), and none tested them together.
FAQ
Does tretinoin go before or after urea?
Tretinoin goes on at night and the urea moisturizer in the morning, which is how the label frames moisturizer use (tretinoin label). If your prescriber lets you moisturize at night too, tretinoin goes first on dry skin. No study has tested the order.
How long should I wait between tretinoin and urea?
Using them morning and night spaces them about twelve hours apart. No shorter interval has been studied.
Can I use a 20% urea cream with tretinoin?
Not on the same skin without your prescriber's okay. Above 10%, urea exfoliates and increases penetration (Celleno 2018).
Is urea an alternative to tretinoin for dark neck skin?
In one trial, tretinoin outperformed 10% urea for acanthosis nigricans (Treesirichod 2021).
Can I use urea every night with tretinoin?
Only if skin stays comfortable. Many people use urea two or three nights a week, or on body skin, while tretinoin runs on the face.
Should urea go under or over tretinoin?
Neither in the same thin layer. Prefer separate nights, or urea on dry patches after moisturizer once tretinoin has settled per your label.
Does urea replace moisturizer with tretinoin?
No. Urea is a humectant and keratolytic helper. You still want a plain cream and morning sunscreen (tretinoin label).
Where does this sit in a full routine?
Night actives on dry skin, moisturizer after your label's wait, sunscreen by day. Map: /layering-order/.
