Can you use tretinoin with clindamycin?
The chemistry
Tretinoin clears and prevents the microcomedones from which all acne lesions grow, while clindamycin is a topical antibiotic that lowers C. acnes and has anti-inflammatory effects. Together they address both non-inflamed and inflamed lesions. A fixed-dose clindamycin 1.2 percent and tretinoin 0.025 percent gel outperformed either ingredient alone in two large randomized trials (Schlessinger 2007). The catch is antibiotic resistance. Guidelines advise that topical antibiotics should not be used alone and should be paired with benzoyl peroxide to reduce resistant bacteria (Reynolds 2024, Zaenglein 2016).
Evidence
Large RCTs back the fixed combination (Schlessinger 2007).
Resistance
Pair with benzoyl peroxide; avoid long stretches of antibiotic alone.
Irritation
Mostly from tretinoin: dryness and peeling in the first weeks.
Common mistakes
A frequent mistake is staying on clindamycin indefinitely. Most guidance aims to limit antibiotic courses and maintain clear skin with a retinoid afterwards. Another is applying the gel only to visible spots; it should cover the whole acne area thinly. Clindamycin and benzoyl peroxide can go in the morning with tretinoin at night, a split many prescribers use since tretinoin can degrade with benzoyl peroxide in the same layer (Martin 1998).
Your routine
Morning
- Gentle cleanser
- Benzoyl peroxide (if prescribed)
- Moisturizer
- Sunscreen SPF 30+
Evening
- Gentle cleanser, pat dry
- Pea-sized tretinoin and clindamycin gel
- Moisturizer if dry
Go slower if
Tretinoin is avoided in pregnancy. Rarely, topical clindamycin has been linked to colitis; report severe diarrhoea to your doctor. Start every other night if you are dry or sensitive.
Evidence strength
Strong. Randomized trials support the combination (Schlessinger 2007), and acne guidelines recommend a retinoid plus antimicrobial approach (Reynolds 2024).
FAQ
How long should I use clindamycin?
Follow your prescriber. Many aim for about 12 weeks, then continue with a retinoid.
Can I also use benzoyl peroxide?
Yes, ideally in the morning or as a wash, to reduce resistance.
Why is my skin peeling?
That is tretinoin. Moisturize and reduce frequency temporarily.
Is the combined gel better than two separate products?
It is simpler, which usually improves how consistently people use it, and that matters more than small formulation differences. Separate products let your prescriber adjust each one.
Can I use moisturizer with it?
Yes. A plain, fragrance-free moisturizer before or after improves tolerability without reducing benefit in practice.
Do I need to wait between clindamycin and tretinoin?
Not with the fixed gel, which puts both in one product. Its label says to apply a pea-sized amount to the whole face once daily at bedtime after gently washing, and not to apply it more often, because extra use adds irritation without faster results (Ziana label). The trials supporting the pair used that single gel (Schlessinger 2007). With two separate prescriptions, no waiting interval has been tested; apply clindamycin first, let it dry, then tretinoin, unless your prescriber says otherwise.
