The chemistry

Tranexamic acid is the better-studied ingredient for discoloration. A 2024 meta-analysis pooled 22 randomized trials with 1,280 melasma patients and found it reduced severity scores, most with oral use, then injections, then topical products, with skin irritation among reported side effects (Calacattawi 2024). A 20-person split-face trial found 3% tranexamic acid cream similar to 4% hydroquinone after eight weeks (Yasnova 2024). Mandelic acid is an alpha hydroxy acid whose best human data are for acne peels: in 50 patients, six in-clinic sessions of a 45% mandelic peel matched a 30% salicylic peel over 12 weeks and caused fewer adverse effects (Dayal 2020). Hydroxy acid reviews list pigmentation disorders among their dermatologic uses (Kornhauser 2010), but no trial has combined mandelic acid with tranexamic acid.

Chemistry

No known reaction between the two acids, which work in different ways.

Irritation

Mostly from mandelic acid. Tranexamic acid can sting on irritated skin.

Order

Mandelic acid first on dry skin, then tranexamic acid, then moisturizer.

Common mistakes

Many people treat dark patches aggressively, using an acid every night because they want faster fading. Irritated skin is harder to treat, so give the mandelic acid rest nights. Results from professional mandelic peels at 45% also do not tell you what a home serum will do. And since hydroxy acids raise sun sensitivity (Kornhauser 2010; FDA AHA page), skipping sunscreen would work against the very pigment you are trying to fade. Keep a simple log of photos taken in the same window light every four weeks, because slow fading is easy to miss in a mirror and easy to overestimate after one good day.

Go slower if

Melasma that spreads quickly, or a single dark spot that changes, should be seen by a dermatologist. Pause the mandelic acid if you get lasting burning or peeling.

Evidence strength

Limited for the pair. Tranexamic acid has a meta-analysis of trials (Calacattawi 2024); mandelic acid evidence comes mostly from in-clinic peels (Dayal 2020).

FAQ

Is mandelic acid good for melasma?

There is no melasma trial for mandelic acid among our sources. The Dayal trial was in acne.

How strong was the tranexamic acid in the hydroquinone comparison?

3% tranexamic acid cream, against 4% hydroquinone, over eight weeks (Yasnova 2024).

Can I use them on the same night?

Yes. Apply the mandelic acid, let it dry, then the tranexamic acid serum.

Do I need sunscreen with mandelic acid?

Yes. Hydroxy acids make skin more sensitive to the sun (Kornhauser 2010).
