Can you use hydroquinone with moisturizer?
The chemistry
Hydroquinone lightens dark patches by slowing melanin production and is used for melasma under medical care. In two 8-week randomized trials with 641 adults, a triple cream of hydroquinone 4%, tretinoin 0.05% and fluocinolone acetonide 0.01% cleared facial melasma more often than two-ingredient versions (Taylor 2003). That product, Tri-Luma, is applied once daily at least 30 minutes before bedtime after washing and patting dry; the label says patients may use moisturizers and cosmetics during the day, and patient counseling notes that a moisturizer may go on in the morning after washing because the cream is drying (Tri-Luma label). Erythema, peeling, burning and dryness were common in the trials (Taylor 2003). The FDA states there are no legally marketed OTC hydroquinone skin-lightening drugs in the U.S.; Tri-Luma is the only FDA-approved hydroquinone product and requires supervision (FDA OTC hydroquinone). A pregnancy review flags relatively high systemic absorption of topical hydroquinone (Bozzo 2011). Exogenous ochronosis is a rare blue-black darkening that means stopping the drug (Ishack 2022; Tri-Luma label).
Night
Thin film of hydroquinone cream as prescribed, often 30 minutes before bed for Tri-Luma.
Day
Moisturizer and SPF 30+; Tri-Luma allows daytime moisturizers.
OTC status
No legal OTC hydroquinone lighteners in the U.S. (FDA OTC hydroquinone).
Common mistakes
Buying unapproved OTC lightening creams online ignores the FDA's warning letters and ochronosis risk. Layering a second retinoid or acid on top of Tri-Luma the same night stacks irritation the label already lists as common. Skipping morning moisturizer because the night cream feels rich leaves dryness untreated; the label invites a morning cream. Stopping sunscreen while spots fade is another setback: melasma returns with small amounts of sun, and the label requires SPF 30 and protective clothing by day. Thin hydroquinone on clean dry skin as labelled, then moisturizer if you need comfort; daytime SPF 30 and clothing are mandatory for melasma plans (Tri-Luma label; FDA HQ OTC). Moisturizer does not replace sun protection; pigment returns with small UV doses. Keep acids and retinoids under prescriber guidance when they share a formula or routine with hydroquinone. Your routine
Morning
- Mild cleanse
- Plain moisturizer
- Broad-spectrum SPF 30+, protective clothing
Evening
- Gentle wash, pat dry
- Hydroquinone cream as prescribed (Tri-Luma: at least 30 minutes before bed)
- Skip extra acids or retinoids unless your prescriber planned them
Go slower if
Use only as prescribed. Avoid hydroquinone in pregnancy or breastfeeding unless your clinician approves (Bozzo 2011; Tri-Luma label). Stop and call your prescriber if skin turns blue-black, blisters or swells (ochronosis and allergy warnings on the label).
Evidence strength
Strong for Tri-Luma efficacy and for label directions that allow daytime moisturizer (Taylor 2003, Tri-Luma label). Strong for the U.S. OTC status warning (FDA OTC hydroquinone). Order of a separate night moisturizer under plain hydroquinone alone has not been compared in a trial.
FAQ
Can I moisturize while using hydroquinone?
Yes. Tri-Luma's label allows moisturizers during the day and suggests one in the morning after washing.
Should moisturizer go before or after hydroquinone at night?
Follow your label. Tri-Luma is applied to clean dry skin at night; keep other products for daytime unless your prescriber says otherwise.
Is drugstore hydroquinone still OK?
The FDA says OTC hydroquinone lighteners are not legally marketed in the U.S.; talk to a clinician about prescription options (FDA OTC hydroquinone).
Why do I need sunscreen with hydroquinone?
Melasma darkens with sun, and Tri-Luma requires SPF 30 and sun avoidance by day.
How long after hydroquinone before moisturizer?
A few minutes, until the thin film has settled. No longer clinical gap is required beyond the label's own directions.
Can I use hydroquinone every night with moisturizer?
Follow your prescription or OTC label. Moisturizer can go on after each use if skin feels dry.
Morning routine on hydroquinone?
Moisturizer if needed, then broad-spectrum SPF 30+, plus hat or shade for melasma-prone skin.
Where does this sit next to other brightening steps?
Ask your prescriber before stacking vitamin C or acids the same night. Site map for non-prescription order: /layering-order/.
