Can you use benzoyl peroxide with clindamycin?
The chemistry
Clindamycin is a topical antibiotic that reduces acne bacteria and inflammation. Used alone, bacteria can become resistant. Benzoyl peroxide kills the same bacteria by oxidation, a route bacteria do not develop resistance to, so pairing it with clindamycin lowers the risk of antibiotic resistance (Zeichner 2012). The 2024 AAD guideline recommends benzoyl peroxide with topical antibiotics rather than antibiotics alone (Reynolds 2024). Benzoyl peroxide is also generally considered acceptable in pregnancy with limited absorption, whereas medication choices in pregnancy need a doctor (Chien 2016).
Chemistry
Fixed-dose gels contain both, stable when stored as directed.
Irritation
Benzoyl peroxide causes dryness and can bleach fabric; clindamycin is usually mild.
Timing
Fixed combination gel once daily, usually at night, or as prescribed.
Common mistakes
The most common slip is using the clindamycin portion on its own for months, for example when one tube runs out before the other. That is the scenario guidelines warn about because it encourages resistant bacteria. Another is using too high a concentration of benzoyl peroxide: 2.5% is often as effective as 10% with less dryness. People also forget that benzoyl peroxide bleaches fabric; apply it at least a few minutes before bed and let it dry. Finally, do not stop at the first sign of improvement without asking your prescriber, as acne often returns.
Your routine
Morning
- Gentle cleanser
- Moisturizer
- Sunscreen SPF 30+
Evening
- Gentle cleanser, dry skin
- Clindamycin/benzoyl peroxide gel, thin layer
- Moisturizer
Go slower if
Clindamycin is prescription-only; follow your prescriber. Benzoyl peroxide bleaches towels and pillowcases, so use white linens. Stop and seek care if you have severe diarrhea while on clindamycin, a rare effect noted on labels.
Evidence strength
Strong. Guideline recommendation plus reviews of combination acne therapy (Zeichner 2012, Reynolds 2024).

Why not just use clindamycin alone?
Antibiotics alone breed resistance. Benzoyl peroxide prevents that.
Can I add a retinoid?
Yes, under guidance; a triple combination with adapalene exists.
How soon will it work?
Many people see improvement within 4 to 8 weeks.
Why not just use clindamycin alone?
Guidelines advise against antibiotic monotherapy because acne bacteria develop resistance; benzoyl peroxide kills bacteria by oxidation, which bacteria do not become resistant to, and it lowers the chance of resistant strains emerging (Reynolds 2024, Zaenglein 2016).
How long until I see results?
Most trials measure at 12 weeks. Expect fewer new inflamed spots by weeks 4 to 8 and do not judge it in the first two weeks, when dryness is at its worst.
Will benzoyl peroxide bleach my things?
Yes. It bleaches fabric, so use white pillowcases and towels and let it dry fully before your face touches anything.
